Question about baby immunization record?

I do not know if this has happened to anyone but my daughters 18 month visit is about to be scheduled and I went to grab her immunizatiion record and I couldnt find it. We recently moved this year but I swear I had kept it with her medical card which is in her diaper bag at all times (It is in there) It seems that something has happened to it or maybe my daughter took it out and lost it. I feel bad about it and I need to know what I can do? Will they replace it? She was at Sharp for on month and Kaiser the rest. Will both hospitals do a replacement for me? Usually they stamp them so I do not know if they will be able to give me a new one with it stamped by the same doctor

HAs this happened to anyone?

Yes, I lost my older’s daughter’s original immunization card and it can was replaced by her current pediatrician. The office can get her old records and transcribe any previous shots with dates. Since, she is young, finding out dates should not be too difficult.

I had to have “official” immunization records for my daughter’s various schools. I contacted her doctor’s office and they printed one right up. They should have all that data in a personal where they can reprint it without a big deal.

This has happened to me. Usually you can call the doctor’s office and make a request for the records, or you can fill out a written request form at the new doctor’s office and they can acquire the needed records for you. If you have her medical records, the doctor’s office may be able to just make you a new copy. I would call and ask the doctor’s office.
